Enrollment Forms:

A signed Enrollment Form for the desired year or summer program and the enrollment fee are required to either enroll or place your child on a waiting list. This form is available upon touring the school, or by contacting the main office at 303-322-8324 x 29 or admissions@mchdenver.org.
